> My mom has epilepsy and sometimes has tonic-clonic seizures. I'm her caregiver and I want to make sure I know what to do if she has one. Can you walk me through the first-aid steps? I've heard conflicting things about holding her down or putting something in her mouth.

For a tonic-clonic (grand mal) seizure, the main goals are **safety and comfort**. Do **not** hold her down, and do **not** put anything in her mouth.
**What to do**
- Stay with her and keep her safe.
- Clear hard or sharp objects away. Cushion her head if possible.
- If she is on the floor, turn her onto her side (recovery position) if you can do so safely, to keep her airway clear.
- Loosen tight clothing around her neck.
- Do not restrain her limbs or try to wake her forcefully.
- Do not put anything in her mouth—no water, no pills, no fingers.
- Time the seizure. Note when it starts and ends.
- After the seizure, she may be confused, drowsy, or disoriented for minutes. Keep her safe and watch her breathing.
**When to call emergency services**
- Seizure lasts more than 5 minutes
- She has repeated seizures without fully waking between them
- She has trouble breathing or stops breathing
- She is injured or you cannot safely keep her safe
- It is her first seizure or a new pattern
Follow her personal seizure action plan if she has one. If she has a prescribed rescue medication, follow her care plan—but do not give food or drink during or right after a seizure. If unsure, call emergency services.

> That makes sense, thank you — so no holding her down, nothing in her mouth, and time it. Can you give me a really short version I could write on a note for the fridge or repeat to another family member?

**Fridge note:**
- Don’t hold her down.
- Don’t put anything in her mouth.
- Clear hard objects.
- If on the floor, turn her on her side if safe.
- Time it.
- Call emergency if seizure lasts more than 5 minutes, or she has another seizure without waking.
- Follow her seizure action plan.
**Say to another family member:**
“Don’t hold her down or put anything in her mouth. Clear hard objects. Turn her on her side if safe. Time it. Call emergency if it lasts more than 5 minutes or she has another seizure without waking.”
