### What this PR does / why we need it?
As #2947 describe, we need to transpose kv cache layout after GQA kv
transfer when prefill and decode tensor parallel size are heterogeneous,
in the previous implementation, we use `npu_paged_cache_load ` +
`tranpose` + `_npu_reshape_and_cache` to do this work.
But obviously, it is not an efficient plan, the ops above need to be
called for each layer, which introduces 3 * layer_num kernel launch, and
6 * layer_num data movement between L1 Cache and HBM for one request on
decode node. Usually, decode node uses graph mode, so these op kernels
will be called between decode forward launched by an async thread in
mooncacke connector, this kernels maybe last for several decode forward
and TTFT will increase by 3~4 decode forward time.
In this PR, we implement an AscendC fused op
`transpose_kv_cache_by_block` to do this with only once kernel launch
and move data between L1 Cache and HBM only once.
After using this fused op, the time cost in transpose kv cacke layout
can be decreased to 0.24ms from 7ms in UT on 910C, and in PD
disaggregation scenario, TTFT can decrease about 90 ~ 110 ms in
qwen3-235B.

### What this PR does / why we need it?
This PR removes the custom `ProfileExecuteDuration` utility and its
usages across the codebase. This utility was used for profiling
execution duration of different stages in the inference process. It is
replaced by the standard `vllm.v1.utils.record_function_or_nullcontext`,
which integrates with PyTorch's profiler.
This change simplifies the code by removing a custom implementation in
favor of an upstream utility, improving maintainability. Associated
documentation and tests for `ProfileExecuteDuration` are also removed.
### Does this PR introduce _any_ user-facing change?
`VLLM_ASCEND_MODEL_EXECUTE_TIME_OBSERVE` env is removed now.
### How was this patch tested?
CI passed. The changes are a cleanup and replacement with a standard
utility. Existing tests cover the functionality. The removed feature had
its own tests which are also removed.

### Motivation.
**Limitations of the current vLLM v1 scheduling strategy**
vLLM v1 scheduling currently enables chunkedprefill by default, which
processes prefill and decode requests simultaneously in a single
scheduling session. This can impact the overall system throughput and
performance in some scenarios.
Balance scheduling addresses this issue by synchronizing the number of
running queues across all schedulers to delay the scheduling of new
requests, thereby improving the overall system's steady-state decoding
time. This achieves:
✅Adding `balance_gather` to the scheduler synchronizes the number of
requests in the running queues between DPs.
✅Balance scheduling improves the decode steady-state time, thereby
increasing the overall output throughput of the inference system.
### Proposed Change.
**1.Feature Overview**
In the vLLM scheduler, running requests (i.e., requests that are already
undergoing pre-filled computation) have the highest priority, followed
by waiting requests (i.e., requests that have not yet been computed).
As shown in the diagram above, when the entire inference system exits
from a steady state, the scheduler will schedule a batch of new requests
for prefill operations and then synchronize them among the dynamic
programming (DP) models. This can cause some DP models that are entirely
decoded to synchronize with the number of prefilled tokens. Frequent
prefill scheduling by certain DP models can lead to a deterioration in
the overall system output throughput.
Balance scheduling synchronizes the number of running queue requests
across different DPs, and only schedules new requests for prefilling
when at least every scheduler has fewer than max_nun_requst.

- Fixed-length input scenario: In the performance test scenario with
3.5K fixed-length input and 1.5K fixed-length output, the throughput
performance was improved by approximately **18%** after adding balance
scheduling.

### What this PR does / why we need it?
This PR adds model-side integration for the previously introduced
experimental AscendC fused operator DispatchGmmCombineDecode, used in
MoE decoding.
The operator implementation itself was added in a prior PR[#4139
](https://github.com/vllm-project/vllm-ascend/pull/4139).
This change only adapts the model execution path to optionally use the
fused operator.
When the environment variable VLLM_ASCEND_ENABLE_FUSED_MC2=2 is set, the
original MC2 path composed of multiple operators (A8W8 dispatch → GMM →
SwiGLU → GMM → combine) might be replaced by the single fused operator
DispatchGmmCombineDecode.
By default, the existing multi-operator MC2 implementation is preserved.

2: enable nz as long as possible;
And `VLLM_ASCEND_ENABLE_NZ`=1 by default.

### What this PR does / why we need it?
Currently, there are two paths to judge the chip type in code,
`get_ascend_soc_version` use `get_soc_version` api in torch_npu, and
`is_310p` `use _build_info.__soc_version__`, which generate when
install. We need to unify the two paths.
We need to unify these codes based on the following points:
1. We need to ensure consistency in chip type judgment between compiling
and running states;
2. In compiling state, we need chip type to complete op's compilation,
but in running state, we only need device
type(910B/910_93/310P/910_95/etc) to make code branch judgement;
3. In compiling state, torch_npu may not have been installed yet, so we
can't use torch_npu's api.
Based on the above points, we have made the following changes:
1. When user set env `SOC_VERSION`, use it; when not set, query
soc_version by `npu-smi`;
2. generate device_type based on soc_version when compiling, and write
`__device_type__` instead of `__soc_version__` in `_build_info.py`;
3. In running state, use `__device_type__` to judge code branch.

When not set env `SOC_VERSION`, it will not be `ASCEND910B1` by default,
we will query soc_version by `npu-smi`. And env `SOC_VERSION` must be in
the list `soc_to_device` in `setup.py`.

Supports generalized FlashComm2 optimization, which reduces
communication overhead, decreases RmsNorm computation, and saves one
AllGather step by replacing Allreduce operations in the Attention module
with pre-AlltoAll and post-AllGather operations (used in combination
with FlashComm1). This feature is enabled during the Prefill phase and
is recommended to be used together with FlashComm1, delivering broad
performance improvements, especially in long sequence scenarios with
large tensor parallelism (TP) configurations. Benchmark tests show that
under TP16DP1 configuration, it can improve the prefill performance of
the DeepSeek model by 8% on top of FlashComm1.

### What this PR does / why we need it?
Currently, when executing to the Linear layer of models in vLLM-Ascend,
the weights format is ND in unquantized case and skipped ascend case.
This PR supplements the execution logic for Linear layer. We use a new
global variable: VLLM_ASCEND_ENABLE_NZ. When VLLM_ASCEND_ENABLE_NZ=1 and
CANN version is 8.3, the weights of the Linear layer will be converted
to FRACTAL_NZ, in both unquantized case and skipped ascend case. We also
use VLLM_ASCEND_ENABLE_NZ to control the existing NZ conversion, such as
w8a8-quantized case.

Add a new global variable VLLM_ASCEND_ENABLE_NZ. If you want to use NZ
format, you should set VLLM_ASCEND_ENABLE_NZ=1.

### What this PR does / why we need it?
This PR addresses a critical issue where Node D (Device) failures cause
Node P (Processor) to hang due to inability to release KV cache.
**Trigger Scenarios:**
1. Node D fails mid-inference (e.g., network disconnection)
2. Node D rejects requests at a certain stage (e.g., via API server)
3. Load-test script termination causes Node P or D to abort queued
requests
**Root Cause Analysis:**
1. Currently, Node D sends a "KV cache pull complete, release approved"
message to Node P
2. This message is transmitted via the worker connector. If PD
connection breaks or requests are rejected upstream, Node D cannot send
the message
3. Node P will never release KV cache without receiving this message
**Solution:**
Following VLLM community's approach (NIXL connector timeout mechanism),
we're implementing:
- A timeout mechanism with comprehensive warnings
- Updated README documentation
- Reference: VLLM's optimization PR

### What this PR does / why we need it?
In vllm version 0.10.1, a new KVOutputAggregator was added to the
executor, moving aggregation to the
executor(https://github.com/vllm-project/vllm/pull/19555). This caused
mooncake_connector to break. This change aims to fix this bug and also
adds a policy to forcibly release the KV cache when the prefill node
times out.
This PR is currently linked to a PR in vllm
(https://github.com/vllm-project/vllm/pull/23917). The vllm PR aims to
modify the finish and send count confirmation in heterogeneous TP
situations.
The reason for deleting many UTs is that a lot of communication codes
have been deleted, so the UT as a whole will appear more concise.

For dense models, by not applying tensor parallelism (TP) to the
attention module and applying TP to the MLP module, the allreduce
operations in the attention module can be eliminated, thereby reducing
computational overhead. However, this approach increases memory usage,
so the environment variable VLLM_ASCEND_ENABLE_MLP_OPTIMZE is used to
control this optimization.

Refactor Sampler implementation from patch way to inherit from vLLM

### What this PR does / why we need it?
it'll execute allreduce and malmul seperately in vllm RowParallelLinear
forward funcion, this function use torch_npu.npu_mm_all_reduce_base to
execute allreduce and matmul in a fused kernel way. this will gain a 20%
performance
promotion in eager mode.
### Does this PR introduce _any_ user-facing change?
this PR introduce a new env `VLLM_ASCEND_ENABLE_MATMUL_ALLREDUCE` to
control whether enable the feature or not.
### How was this patch tested?
the patch is tested by adding a new test file `test_patch_linear.py` to
guard the ut
